Target: Older adults aged 65+ with pre-frailty or frailty, living in the community
Focus: 1) Physical function improvement (strength, balance, flexibility); 2) prevention of frailty progression; 3) empowerment through digital self-management tools; 4) integration of physical activity, nutrition, and remote monitoring
Type of prevention targeted: Secondary (early detection and intervention in pre-frailty to avoid progression) and tertiary prevention (slowing or reversing frailty progression to reduce disability and dependency)
Objective: Evaluate the effectiveness of a 12-month digital, home-based intervention combining physical exercise, nutrition guidance, and health monitoring in preventing the progression of frailty and supporting autonomy in older adults. The study also aims to explore feasibility, usability, and adherence to a personalised digital care model.
