One major limitation in studies targeting frail older adults is the lack of a defined frailty syndrome, hindering accurate patient identification. This study addresses this gap by focusing on the five components of the frailty syndrome: unintentional weight loss, self-reported exhaustion, weakness, slow walking speed, and low physical activity levels, as defined by recent research.A co-creation phase involving 25 participants will refine a digital solution, followed by a six-month pilot study with 50 additional participants to test and improve the app/platform prototype. The main pilot study will include over 230 participants, assessing them at baseline, three months, and 12 months. Participants will use monthly calendars to record falls, hospitalizations, weight loss, and other events, with follow-up for incomplete or incorrect entries.
